Claim/Complaint Form
The main form to start your case. Lists your claim amount, what happened, and who you're suing.

Summons
Official court document notifying the defendant of the lawsuit and hearing date.
→ Often provided by the court after filing
→ Must be served on defendant properly

Proof of Service
Document proving you properly notified the defendant about the case.
→ Must be completed by the person who served papers
→ File before your hearing date
→ Keep a copy for the hearing
Fee Waiver Application
Request to have filing fees waived if you can't afford them.
→ Must show financial hardship
→ Submit with your initial filing
→ Include any required supporting documents
Postponement Request
Request to change your hearing date if you have a conflict.
→ File as soon as you know of the conflict
→ Include specific reason for request
→ May require other party's agreement
Subpoena
Order requiring a witness to appear or produce documents.
→ May require court approval
→ Must be served properly
→ Use sparingly - judges prefer cooperation
Tips for Filling Out Court Forms
Type or print clearly in black ink
Illegible forms can delay your case. Many courts prefer typed forms.
Complete every field
If something doesn't apply, write "N/A" rather than leaving it blank.
Use exact legal names
"John Smith" vs "John A. Smith" matters. For businesses, use the full legal name including Inc., LLC, etc.
Make multiple copies
You'll typically need: one for the court, one for each defendant, and one for yourself.
Sign where indicated
Don't sign until you've reviewed everything. Some forms may need notarization.
